Fortis Health Insurance has changed their name to Time Insurance. Please click on the Forms Link to download the updated application forms.
Student Health Insurance Products are available in states except New York.
We represent Fortis in: IN, MD, OH, PA, SC, TX, VA, WI and WV.
On Assurant's Website, you can download the Application Forms or enter an Application online. (A Login is required). The Forms are also available here on our site, without a login.
