Fortis is now Time Insurance.
Time Insurance's Short Term Health Insurance Product is available in states except Hawaii.
We represent Fortis in: IN, MD, OH, PA, SC, TX, VA, WI and WV.
On Assurant's Website, you can download the Application Forms or enter an Application online. (A Login is required). The Forms are also available here on our site, without a login.
Please note: Check for state availability for each carrier as not all plans are available in all states.
